School Health Forms

Below are some forms that you may want to download when your child has a doctor, dentist or eye doctor appointment.  Please have the doctor complete these forms and return them to me as soon as possible.  If, for any reason,  your child needs medication at school, there is also a form (located below) that needs to be filled out by their doctor.

   

Health History Form:  To be completed by the parent at registration and as needed.

Promotion of Health:  To be completed by the child's doctor.  We recommend that students entering the first, fourth and seventh grade, as well as new students, have an exam by a physician.

     Dental Health Form: To be completed by the child's dentist.  We recommend that students in kindergarten                                                                                                  through grade five have a dental screening at least yearly.
